Abstract
Parkinson’s disease (PD) is a progressive movement disorder due to a loss of dopamine production in the brainstem. The four primary motor symptoms include a tremor, muscle rigidity, bradykinesia, or postural instabilities (NIH, 2023). Non-motor symptoms including depression, anxiety, apathy, orthostatic hypotension, sleep disorders, and cognitive impairments can also impact individuals with PD (Parkinsons.org, 2023). Motor and non-motor symptoms of PD impact an individual’s occupational engagement and self-efficacy for ADL’s. Overall, the quality of life for a people with PD and their caregivers is greatly impacted. Nearly 1 million people are living with PD and this number is projected to rise to 1.2 million by 2030. It is the second most common neurodegenerative disease following Alzheimer’s disease. In the United States, indirect and direct costs of PD including lost income, treatment, and social security payments is roughly $52 billion per year (Parkinsons.org, 2023). The primary risk factor for PD is age with incidence estimates an increased risk with 65 and older age range. The cause of PD is believed to be a combination of genetic and environmental factors and is more common in men than women (Parkinsons.org, 2023).
Lee Silverman Voice Treatment (LSVT) BIG is a program for individuals with Parkinson’s that can be provided by a licensed and LSVT BIG certified occupational therapist or physical therapist. It aims to improve movements and engagement in ADL’s by targeting gross and fine motor skills through exaggerated and repetitive exercises. Exercises focus on increasing the size or amplitude of movements. The program is prescribed individually or in groups for 4 times a week for 60 minutes for 4 consecutive weeks. Symptoms of Parkinson’s such as slow movement, tremors, gait disturbance, postural instability, muscle stiffness and decreased balance can be addressed through LSVT Big intervention and in turn promoting an increase in quality of life and occupational engagement. Participation in LSVT Big under supervision of an occupational therapist, improves self-perceived performance of prioritized daily activities and self-care which contributes to functional independence and validating the importance of occupational therapy involvement for individuals with PD (Doucet et al., 2018)
