Abstract
Purpose: This project reviews the literature on factors contributing to delays in new patient appointments, offering evidence-based practice recommendations for nurse leaders. Background: Prolonged wait times for new patient appointments in specialty clinics lead to poor patient outcomes, inefficiencies in health care delivery, lower patient satisfaction, and increased healthcare costs. Methods: A comprehensive literature review was conducted using databases including CINAHL, EBSCOhost, PubMed, and Google Scholar. Key terms such as new patient wait times, bottlenecks in specialty care, access to specialty care, and appointment delays were used. Articles written or published between 2015 and 2024 were included for review. Results: The literature review revealed that delays in new patient appointments are common across multiple specialty areas, and the issue is multifaceted, requiring more than one solution. Conclusion: Clinical practice improvements can be achieved by addressing operational inefficiencies, utilizing Nurse Practitioners (NPs), discharging patients from specialty care when appropriate, and implementing telemedicine and e-consults to improve access to specialists.
Keywords: new patient wait times, bottlenecks in specialty care, access to specialty care, specialist outpatient wait times, process improvement, appointment delays, and cardiology referrals
