In early May, 1974, Mike Hart, the Check Collection Department Head for the XYZ Bank asked Sid Collins, the Check Processing Section Head to meet with him regarding several problems which had begun to surface in the Department. Hart was concerned with the pressure he had been getting from members of senior management regarding the performance and efficiency of the Department and he wanted to prepare a response to these questions.
