Abstract
Purpose
This Doctor of Nursing Practice (DNP) project aimed to implement an evidence-based oral health screening and intervention program for adult patients in a primary care clinic in Lincoln, Nebraska. By including thorough oral health evaluations, instruction, and focused interventions in routine primary care visits, the study sought to enhance oral health outcomes. The projected outcomes of the project included improved oral health for adult patients in the primary care clinic. This entailed fewer oral health issues, increased patient awareness of the value of oral health, improved oral hygiene habits, and increased use of effective oral health interventions such as fluoride applications.
Methods
A twelve-week cross-sectional analysis involved 105 participants aged 19-80. It utilized surveys and oral health assessments, including the Oral Assessment Guide (UNMC) and the American Dental Association's Caries Risk Assessment form. The study employed a theoretical framework for tailored interventions encompassing education, prevention, and fluoride treatments. Participants received personalized education, preventive measures, and fluoride applications as part of the intervention strategy.
Results
The chart review revealed a distribution across age groups, with a predominant male representation. A total of 105 patients underwent oral health screenings, with 43 screening positive for oral health concerns. Among these, five patients accepted and received fluoride treatments as part of the intervention. All patients identified with oral health concerns received comprehensive oral health education, coupled with a curated list of local dentists, categorized by private pay, Medicare, and Medicaid options, ensuring personalized and accessible avenues for continued dental care. Healthcare workers, including physicians and nurses, unanimously rated the oral health protocol as 'very satisfied.’
Implications to Practice
The project's outcomes suggest a positive impact on oral health within the primary care context. Addressing oral health concerns contributes to better outcomes, reinforcing primary care settings as proactive spaces for preventive oral healthcare. Despite cautious recommendations from the U.S. Preventive Services Task Force, the project aligns with the American Dental Association's emphasis on the crucial role of primary care providers in promoting oral health and preventing dental caries (USPSTF et al., 2023; ADA, n.d.).
