Abstract

Abstract Purpose: This review synthesizes the literature on case management in Federally Qualified Health Centers (FQHCs) to assess its effectiveness in improving patient outcomes, reducing healthcare costs, and addressing chronic diseases and social determinants of health (SDOH). Evidence-based practice recommendations for healthcare leaders and case management teams are provided. Background: FQHCs serve vulnerable populations, often facing challenges in care coordination due to high patient volumes and limited resources. Case management has emerged as a key strategy to improve care coordination, reduce hospital readmissions, and enhance patient satisfaction, particularly for individuals with chronic conditions and complex healthcare needs. Methods: A literature review was conducted using databases such as CINAHL, EBSCO, OVID, and Google Scholar, with key terms including case management, healthcare coordination, chronic disease management, and social determinants of health. The review focused on the effectiveness, cost-effectiveness, and barriers to implementing case management in healthcare settings, and included articles published between 2019 and 2024, such as systematic reviews, meta-analyses, and qualitative studies. Results: Case management programs, when supported by appropriate training and resources, have shown positive effects on patient outcomes, including improved disease management and reduced hospital utilization. However, barriers like lack of provider buy-in, limited resources, and insufficient technological infrastructure can hinder their effectiveness. Transformational leadership has been identified as key to overcoming these challenges and ensuring program success. Conclusion: Case management in FQHCs is an effective strategy for improving patient care and reducing healthcare costs. Recommendations include fostering transformational leadership, addressing barriers to program success, and investing in resources and training to ensure long-term sustainability and impact.
